The box below is what I use also. You said the note is disappearing from the title block. I assume you mean it still shows in the Custom Property box? If that's the case, then one possibility is that the note lost it's link to the property. Another is that the note is in a Layer that's turned off. If neither of those is the problem, can you post a drawing? It doesn't need to have any drawing views, just a blank drawing.
How quickly does it disappear? In a couple minutes? As soon as you save? After closing and re-opening the drawing? Off the top of my head I can think of two possibilities:
1. At some point a macro is being run that overwrites or erases that custom property.
2. The drawing is not being saved after the property is added. Make sure the drawing is not read-only and verify it is saving. Just because the drawing view is updated doesn't mean the drawing file itself has saved.
Kind of tough if I can't see the part and drawing - but here is what is possibly happening...
I probably can't explain it right, but here is my take, the information is in two different places, (custom property - configurations property). I've have had issues with this as well. Check the part properties and check where the info is and check your drawing to see where it is linked to.
The link is being blocked or broken...

Thanks for the responses, but I think it's time to bring it to my reseller since I think this is a bug in SW2015.
I have opened older drawings in SW2013 and the problem doesn't exist there.

VAR suggested I do a repair on my SW install... thinks there is something wrong when I did the SP04 update.
Did the repair.... no change, CheckedBy property still disappears as soon as I hit save.
At this point not sure what else to do

Are you able to post a screen shot of the drawing properties page as shown (with all the properties before save)
Are you typing the property and entering the values there by yourself or populated from the property text file?
If it is not populated from somewhere else, that may be one of the reason (I suspect)
WOW.... my VAR figured it out.
Seems there was an attribute in my PDM variables for CheckedBy that didn't belong.
