I'm only replying to be on this discussion. I'd like to know too if someone has a good solution for this.
From my experience the balloons are not shown on parts that are hidden which makes sense because the balloon anchor doesn't have any edges or faces to attach to. So the balloon is not created.

Scott is correct, I would guess that the items missed are hidden.
When I have items that are hidden in a standard view, I change the view to an isometric view, manually balloon the far side components, go back to normal view then change the balloon box to a dashed line so everyone knows there is a part on the far side.
Then you can auto balloon the parts on the near side.

Did you explode the assembly - if you did the balloon call out is based on the bom type you pick, Top level only - will select only the assemblies and top level parts, Parts only will get all parts.
If you don't have the assembly exploded then the guys above are correct....
