BOM missing the first item number

The first item in the BOM is missing and I cannot add it in. Every time I add a new row it just defaults to item number 2. Where has item number 1 gone..?. Is there a switch for this or possibly it is hidden..?? I havent edited the item number column so it should be just following the order.
