why is a coincident mate offsetting and becoming an error?

I have this mated component that is not being restricted by other mates but still wont work. Im trying to make two surfaces coincident but they keep offsetting and becoming "unsolvable". If I de-select and re-select one of the faces it appears to be coincident but then offsets when my second mate entity registers in the dialog box wich you can see in pictures 3&4. Then it doesn't become "unsolvable" until I rebuild, pic 5 shows the "solved" mate still offset. I deleted all other mates in the item that could affect this one but it doesnt seem to have helped. And I updated to SW2015's SP02 to try to fix it and that did nothing either. Has this happened to you before and is there a way to fix it?
