The simplest way would be to place the dimension first, then close out that command and start a note. Place the end of the note's leader and then click to place the text. After doing that, while the note's text box is active (where you'd usually enter the note's text), click on the dimension. That will place the dimension text in the note. After doing this you can place the second leader as you would for any other note, and you can right-click on the dimension and choose Hide from the drop-down. With this method, the note maintains a link to the hidden dimension, so if the model is edited the note will update (although it may take a rebuild).
