Both sketches are parallel with lines horizontal or vertical. I also added guide curves (straight lines) on each edge and middle to middle of each rectangle, but this did not help. How are the sides not flat/planer and how do I get them this way?

Here's a picture. The start/end conditions are set to none. All the connectors are good although not shown when I took the screenshot.
The sides should be planar. How are you determining that they are not? An easy check is to right click on the face and see if you can start a sketch on it.
Fixed it. Even though the lines for the rectangle sketches were set horizontal and vertical, the two sketches were off 0.17 deg. Thanks
What do you have the start/end conditions set to? None (start/end constraint) would be my best guess without seeing a picture of what you're describing. Also you can turn on the connectors to make sure your loft isn't twisting at all. To do this, right click in the user interface and chose show all connectors. You can click and drag those connectors along the sketch segments to change how the loft is created.
