We've lately been getting a lot of "Could not access Item in the Database" it is really scary because the only solution seems to be to create a copy outside of EPDM and the bringing it back in and overwriting the one inside the Vault, this is not good because we loose al History! It randomly starts happening to files that have been OK.

Any Ideas what could be causing this? or if there's a SP that addresses the issue?
