Has this happened to anyone else?
I have never experienced this in any other version of Solidworks, but when I place any form tool on sheetmetal and click 'OK' SolidWorks crashes. Is this just a bug with 2013. If so it'll be very annoying until fixed in SP 1.0.
Have you gotten this resolved? I am experiencing the exact same problem. I sent an Rx to my VAR, and was told it was a .NET Framework issue with instructions from Microsoft on repairing/uninstalling/installing. I went through all of the instructions, uninstalled SolidWorks then re-installed SolidWorks. After all of this, I am still getting the exact same thing. I cannot even go into pre-existing parts with forming tools and edit their forming tool feature without SolidWorks shutting down.
I figured out my problem, but I have no idea why this should matter.
If I use the View/Workspace/Dual Monitor, applying forming tools will crash SolidWorks 2013 every time. If I switch the Workspace to either default or widescreen, they work.
So, I'll bet that you are using the dual monitor workspace.
Update:This is logged as spr 679276: (crash after clicking ok to undocked form tool feature property manager)
I have same issue here, and i found this issue comes in Dell workstations.
Same thing here. I am having the same issue on Dell workstation and Dell mobile workstation. Running SP1.0
Update: Switching from Dual Monitors to Default also worked for me.... Seems like a legit (but very strange) workaround.

This work around fixes the issues for me also.
I run SolidWorks 2013 on a Dell Alienware M17XR4 laptop. I have a second monitor and run dual monitors with the laptop screen selected as monitor 1.
Selecting 'default' allows me to place form tool features without SolidWorks crashing.

This was happening to me as well. I have an HP Z420 Workstation. As soon as I changed the setting to Default it worked. Thanks for the tip!
This problem is still happening with me. I am using a single monitor setup at the moment, and I've tried just about everything to fix it.
Every forming tool crashes Solidworks 2013.
Yes Solidworks or VAR do not need to resolve this issue. They always blah blah......
Root Cause....
When I changed the file location for Form tool or when I add a file location for my cutom forms tool at the option menu the problem arise.
I am totaly unsatisfy with the VAR and solidworks ppl

Are these forming tools from a older version of SW, or are they the ones that come with sw2013?
if they are older versions, open the forming tool and save it so it will be a 2013 file.
