How to create a new Service Request with the SolidWorks Technical Support team?

Hi there!

The problem is that our SNL server is suddenly dead and I need to move the activation to another server. FAQ states that in such a situation I have "to create a Service Request with the SolidWorks Technical Support team". But it is unclear how I should do it. In SolidWorks customer support web-site under my profile it is only possible to browse my already created SRs but there's no option to create a new one.
