I am using SW2011 SP3.
I just attached a balloon to a component in an assembly drawing. This was an existing drawing with a BOM and exploded view already in existance for what that is worth.
The balloon auto-filled with the BOM item number. In the balloon property manager I changed the Balloon Text drop down from Item Number to Custom Properties. The list of custom properties then presented was limited to the drawing. I picked one at random. This seemed to create a new balloon with the custom property and left the first balloon with the Item number. Deleting the random balloon and editing the first placed balloon a second time now populated the custom property list with the component custom properties. Shown below is the components number property.

Thanks for the tip, I never noticed it. Now that I know it I think it will be a great option.
FYI, in regards to your note in trying to link it to the part you would have to
type in "@xxx.prt" at the end of the custom property string to get it to be for a
specific part. Then if you still wanted to go this route you can place a circle border
around the note to make it look like a balloon.
