I don't think so. He is a single monitor user. It happens every time he tries to edit a table.
I have never seen it disappear. Try renaming the CU registry key to see if it reappears. If it does, it's something in those reg settings. Or if you are nervous about editing the registry, you might try starting SW with the Rx option to bypass all the settings and see what you get.

Yes, this is a standard SW table, not an Excel table.
Does it dissapear when he goes into "edit table"? If that is the case, it is probably because SWx puts up the styandard Excel toolbars so that the table can be edited. The way I usually do it is to "edit table in new window". That way, the table is opened in Excel and the SWx toolbars remain.

A colleague of mine had the same issue today.
The position of the "Table Format Dialog" can be locking on a fixed position on the screen using the pushpin.
The "Table Format Dialog" does appear, but outside the visible screen.
1. The "Table Format Dialog" is locked in position with the pushpin when using dual monitors.
When switching from dual monitor use to a single monitor, the dialog is placed on the "other screen" (which is gone).
2. You never used a dual screen setting on you system, but copied settings from a dual monitor setup.

I had this same problem with a dual-monitor setup in SolidWorks 2014 and solved it through editing 3 DWORDS in the Registry. I found in addition to setting the value to unpin the dialog that it was necessary to reset the dialog horizontal and vertical position values so that they were within the range of the primary monitor.
