If you are using SolidWorks 2011, read this short article. The culprit is most likely a simple option setting.
Thanks for the solution, this worked perfectly for me. SW 2011, SP 4.0. Had this problem with custom Unistrut parts. Made them from the toolbox, renamed and saved in a working directory. Built an assembly using several configurations of each type of strut, then used this assembly in a sub assembly, and ultimately in a top level assembly. Closed out the assemblies, and when I reopened, all the parts were wrong sizes and refrenced from the toolbox. Interesting note, when looking at the assembly using SW Explorer, the correct parts were refrenced; but when opening the assembly file, the toolbox parts would be used.
The above checkbox appears to be the culprit, and now the assemblies work properly.
