I don't believe it is anything you have done. It appears to be a bug.
I suggest you submit the file to your VAR.
Yes agree with Kelvin. It is surely some kind of bug. If you suppress the visible flange then in flat pattern you have the one which is not coming earlier. Also I created on more flange and it work OK
