You need to add fillets on the sharp bending edge for flattening the part.

BTW, why didn't you model this as the Sheet metal part itself.
Its actually from a much larger multibody sheetmetal part. I just re-sketched it for simplicity's sake for posting on here. I've incorporated the changes to the original and its all working great now.
