      First of all you're taking the long route to construct a box.
      second, you want to use a planar surface feature to cap the ends, not a surface fill. A surface fill doesn't always come out planar.
      To echo JDMather, after you've created your box faces, use the knit feature to stitch them into a single surface. You don't have to form a solid body, but you can at this point.
      Once the surfaces are knit, you can use the fillet feature with the full-round option to create your vault (see attached).
      Now, if you don't need this to be a surface body, you can draw your base profile, extrude it up, add the fillet and skip the extra 3 steps.
