How do I create the perforated holes?

Use form tool to create this and then pattern that feature. But please be noted that SW currently doesn't have the capability to flatten this kind of geometry in sheet metals.
Have you check the embossed tools?
In Design Library/Forming Tools/ Embosses.
You may have to adapt/create one to suit your requirments but its a good starting point.

If you have 2 configs one with the embosses suppress then you should be able to flatten it.
The sheet metal will get flatten even with the form tool un-suppressed but form tool will not and it will stay there as such.
Sometimes, it's easier to find a supplier and purchase the materials you seek. For a custom fabricator, or job shop to try and duplicate what can be mass produced often can add needless expense to the project.
There are several large companies that make thousands of designs for the tread plate you wish to make. One example:
McNichols is another that comes to mind.
You can sort of model the embossed tread by using an edge flange on the holes. It will unfold, but it is a tedious process. I only completed 1/4 of the work in the attached sample, and even then only put in a few tread features.
