When open/checkout a read only document from vault, creates a local copy. The local copy shows equal symbol. But when you say Update/Reload from Vault, it show an incorrect message, that "the local copy is newer than that of the vault, are you sure that you want to replace this copy with the older one."
though the document is same.

Does anyone else faces the same problem?
It sounds like ypur PDM Options. Go to PDM Options --> Vault --> and make sure that radio box under "Compare vault document" is set to "To document on disk".
