So I'm trying to create a reference plane that is offset in both angle/orientation AND distance from any of the three planes at the origin.
Using SW 2019, it doesn't appear that this is an option. Am I mistaken, or can you only choose one offset type, either just angle or just distance?

AFAIK the only option is to create a sketch on one of the primary planes, then use it as a reference for your plane.
only one type of reference to another plane is allowed.
but, since the angle offset requires at least one more reference than a single plane, you can make your new plane where you want with just the angle option.

My recommendation would be to make a sketch(es) with construction geometry to orient the plane the way you want it if you really need a plane. However, in a lot of cases I've encountered, extra planes can be avoided with use of faces and ordinate planes as sketch planes.
