I want to connect a cylinder/tube with a flat plate and then flatten all of them. The picture below is what I get with non-sheet metal features. But I don't know how to achieve them in sheet metal. 

For the tube and the plate, I can use two base flanges, but they will be two separate bodies, and how to combine them and flatten at the same time?

For the connection, I can use lofted bend but it looks really bad and broken at some places.

I attach the model in sheet metal for reference. Does anyone have some suggestions? Thanks!

Seems there is no perfect solution for it in sheet metal. Finally we make part of the cylinder to be flat so as to connect to the flat plate.
