How do I split the attached box into 5 pieces and turn them into 5 sheet metal parts? I have a 3D sketch that shows where I would like to split the parts.
Split them using Planes and then save out individual parts
I would do a convert to sheet metal and keep body for every piece except the last sheet you convert.

How would I split that top face when I am converting to sheet metal?
I was unable to open your attempt because I have an older version of SW (2017), However, I was able to get it converted to sheet metal part like I wanted. My issue now is that all of the edges are interfering with each other and I need to edit each individual sheet metal part and I am trying to figure out how.

You may check reversing the thickness in your convert-sheet metal features if all the pieces are interfering with each other.
The pictures below kind of show how I went about this but i included a step file so you can open and look. The top face I had to go back and create edge flanges for because the convert to sheet metal tried to grab the whole top face when I collected that bend. Also, some of the flat patterns for these pieces are relatively large. Most readily available sheet sizes are between 4' x 8' and 5' x 12' unless you get a 8' x 20' "master" sheet but those can be cumbersome to cut and get in your average press break.
