I've been getting this issue with my toolbox bolts where the model is changed. I'm not sure what caused this to happen. I opened an assembly drawing and noticed that all my bolts are messed up.
Toolbox folder moved/missing.
System option file location changed.
Toolbox setting changed.
Thank you. File location was not pointing to the correct folder for the current SolidWorks version.
I am having this same issue in SW 2020 SP3. I have the file location looking at the correct location. Solidworks just randomly decides to resize a bunch of my screws for no reason. Everything was fine yesterday. Today all the screws are wrong. The sad thing is I make derived patterns of the screws and they stay the correct size but the leader of the pattern gets resized. From what I am reading on the web and other forums is Toolbox is junk and do not use it. I really like the way it works but if I have to keep redoing the screw every couple days then it is not worth even installing it.
Anyone have any ideas? I am using PDM also.
You have a shared Toolbox in PDM?

Yes it is a shared toolbox. Right now I am the only user that has gone to our PDM system. The other users are not
in PDM and run with a non controlled PDM set up. I am the only user ATM working on this project. I do use 2 different computers (one on site and one offsite) but I remote to the same PDM vault on both computers.
Did that happened after you remote in with another computer and open the assembly? Or worked on something else?
Toolbox setting has to be the same for ALL computers sharing Toolbox.
If you changed default setting in Toolbox, that need to be set on every computers and every time SW upgrade to new version.
Check the file names in the assembly, the leader and pattern.
Open the toolbox item alone and see if its right size.
