To report back, the issue has been solved and Response Surface Viewer works again on my SW2018 SP4.0.
See SW Support reply below:
" After investigation, this issue is due to the SW installer. The program “CqmViewer.exe” is absent in the “C:\Program Files\SOLIDWORKS Corp\SOLIDWORKS Flow Simulation\binCFW” folder after installation."
Please copy the attached executable to this location on the customer machine and try again the surface viewer.
An example of installation path is “C:\Program Files\...\SOLIDWORKS Corp\SOLIDWORKS Flow Simulation\binCFW”
The following SPR (SolidWorks Performance Report) status has changed to Implemented.

Description: Error: 'Response Surface Viewer failed to start' when clicking the View All Response Surfaces button in a Design of Experiments and Optimization study
