I seem to recall seeing something link that years ago and found that they were cosmetic threads that were messed up. Do you have any of those in any of the parts?

Wayne and Josh were right. The cosmetic threads created by the hole wizard were the problem.
Simply opening the feature and accepting it again without changing it solved the problem.
Very interesting, can you zip all files and post to see if this is something with the file or your system?

Those definitely look like screwed up cosmetic threads. They can happen especially when editing the hole location sketch of a hole wizard without editing the hole wizard itself. Mine has always been fixed by editing the hole wizard feature. You don't have to actually change anything, just edit the feature.
If you're able to recreate that send it to SW. We have been reporting this same problem since SW2005 and they don't think there's a problem. Trying to get it to repeat is a real pain as well. We usually don't notice it until well after we're done with the modeling and then open up the drawing and they show up this way.
