Thanks for the reply. I actually reached out to my VAR regarding this as well. The first thing they checked was the conditions I had set to add files to my production vault's workflow. It turns out that I had the same condition applied to an obsolete workflow that we are no longer adding files to. The user had no rights (intentionally) to access files in this old workflow; which was causing the issue.
In order to fix this, we set a condition on the old workflow to look for files that are not equal to anything (!=%). This will stop adding files to the workflow going forward. Then on the files that were stuck in the old workflow, we added workflow links for each duplicate state to the corresponding state in the live workflow. Then I did a search in my live workflow for the stuck files and transitioned them to the live workflow.
The lesson here for me is not to have any more workflows than absolutely needed. This will make things easier to maintain and will not add to any confusion.
