How do you "meld" together several extrusions into a seamless form?

Hello all,

 

I'm currently working on a project and I'm looking to create a melded look with the model. My model currently looks like this:

And I am trying to make it look more like the top of the glove.

It has a seamless transition between the knuckles and a seamless transition from the knuckles down to the hand. I know that I could technically create a spline and extrude the spline to make a look like that, however, I want to keep the dimensions fairly close to what I already have in my model and splines tend to be difficult for me to accurately dimension. I have attached the model as well to this question. How would I go about making this sort of melded/seamless transition between the extrusions I already have? If it is not possible to do it with the extrusions I already have, how would I even go about making this sort of extrusion to begin with?
