We have all our custom items defined and working in 2008.
I thought I recalled seeing something about being able to convert the old DB for use in 2009, or is this wishful thinking?
Also, for custom hole callouts, is there anything special required for 2009 or can I just cut and paste?
Thank you.

This is a responce I recieved from our VAR regarding this issue:
The update to "swbrowser.mdb" can be manually performed by following the steps below:
1. If the user is running a shared Toolbox installation where multiple users interact with the same SWBrowser.mdb file in a shared location, then ALL users must exit SolidWorks to ensure that no user is locking the database. If the database is currently in use, then the update will fail.
2. On a user's machine open Windows Explorer and browse to "\Toolbox\data utilities" and locate the file called "UpdateBrowserData.exe". Double click this file to run the update utility.
3. For the "Updating Database" field browse to "\Toolbox\data utilities\lang\English" and choose the file "updatedb.mdb".
4. For the "Database to Update" field browse to "\lang\English" and choose the "SWBrowser.mdb" file at this location. In the previous mentioned path the , if unknown, can be determined by checking Tools->Options->Hole Wizard/Toolbox.
5. The "Current Version Database" should remain empty and the "Log Filename" field does not need to be altered.
6. Click "Update" to run the update process and that should result in the database being migrated to the newer version.
You may have to run a query for "UpdateBrowserData.exe" to find its location. If you are sharing a toolbox on a server, then it only has to run once. If everyone is using a toolbox on their computers, then this will have to be run on each client. Yes, this is installed with each version. I found mine under C:\Program Files\SolidWorks Corp\SolidWorks\Toolbox.
I upgraded from 2008 SP3.1 to 2009 SP2.0. I also wanted to retain all of the customizations we had made to our Toolbox and its files. We run a shared environment and after running this everything converted over nicely with no issues that I am aware of.
I hope this helps.
