What I observe is that cosmetic thread display is controlled by 3 settings.
to display cosmetic thread,
1. View All Annotations --> set 'ON' (controls global display of annotations)
2. Cosmetic threads --> checked in display filter of annotation properties
3. Display annotations is checked
if anything is missing cosmetic threads will not show up (in 3D).

and also make sure "Display Annotation" is checked.
Where does the need to "Display Annotations" come from with regard to correctly displaying cosmetic threads?

Thanks for reminding me Dave ,, The Shaded cosmetic threads can also be accessed by right clicking on "Annotations" and selecting Details.

Cosmetic threads are part of annotations. When "Display annotations" is unchecked from feature tree all the annotations including cosmetic thread gets hidden. I think OP has created correct cosmetic thread but it is not showing up in 3D model. (I've 2015, cannot open his model. My judgement is based on his screen-shot). You can check for yourself. After you create cosmetic thread --> Hide/Show annotations.Before you do that make sure you're displaying all annotations under view --> All annotation
I truly wasn't aware that cosmetic threads were controlled by Annotations like that!
I haven't tried it, as I don't use cosmetic threads, but interesting information just the same.
