Ask for a sample first - but like Tony Cantrell mentioned - shouldn't be a concern for most shops with good brake press operators..
I would use two fit bottom dies and one top punch that would span those bends. Don't dimension to the top of the relief just give a dimension for the width of the relief slots on the sides and say "TYP". Their bend deductions 'could' move that. Even though I would bend both at once you never know what a shop will do.
We do the exact same thing except we don't use the side relieves.Since we use laser cuts a slit suits us and the width of the bend is the same as the relief.
