When I try to log in to my vault there is an error "Could not log in to the database on server Cause: Object already exists "?

I have searched in Knowledge base but could find a solution.

Also when log in to the vault in Administration Tool it shows me the message that "Administration tool stops working" which is very unusual.

If anybody familiar with these problems please provide a solution.
