Sketch Point has two positions in drawing template and sheet format?

This is doing my head in. I have a sketch point in a drawing template and another point drawn in the sheet template. Let's assume these two points are coincided (for the sake of my question). For some reason both points have different x and y. Is this due to the fact the sheet format sketch has a different origin to the sheet template? How to transform between both?

 

I tried get to the MathTransform (ModelToSketchTransform method) from the point's sketch and I got this array:
1 0 0 0 1 0 0 0 1 621.042779996684 569.569675290997 0 1 0 0 0

 

SolidWorks does not tell you how this array is parsed into a 4x4 matrix.

 

So I'm assuming it should be like this?

1 0 0 0

1 0 0 0

1 621.042779996684 569.569675290997 0

1 0 0 0
