SW crash when checking in or out of vault

Along with other issues, the Vault is causing me problems.

info:
SW 2008 X64 SP 4.0
Windows XP Professional x64 V2003 SP 2
Dell Precision T7400
Intel Xeon
E5430 @ 2.66ghz
8gb ram
NVIDIA Quadro FX 4600

50% of the time, any type of access to the vault results in a crash of SW. Checking in, checking out, loging in or out of the vault makes SW go POOF. Often no error message.
