This is working okay for me using SW08 SP0.0, see attached image.
My problem continues to be "tolerances".
Note that the Ø.096 has a ±.005 tolerances but the corresponding mm dimension, 2.44, does not display the proper tolerance as [2.44±.12]

It's the text that is not showing ( in or mm). When you do a linear dimension, the "in" and "mm" are shown with the numerical values.
SolidWorks is lacking in this area and they do not meet the requirements of ASME Y14.5M, that states; Where a combination of units are used, imperial dimensions shall be followed by the abbreviation "IN." and Metric dimensions shall be followed by "mm".
Per drawing standards, your drawing format should state;
UNLESS OTHERWISE SPECIFIED
DIMENSIONS ARE IN INCHES
When we use Dual Dimensions we add a caption right above our tolerance block as follows;
DIMENSIONS SHOWN IN BRACKETS
ARE IN [MILLIMETERS].
You may want to do the same.
