Flat folding layout to 3D

Hi, I'm just started studying Solidworks and 3D in general and now I have a problem.
I have a "flat" layout how to fold a steel plate into a paper forming shoulder.
The shoulder will take a flat paper 29 mm wide and form in to a tube with 8 mm diameter and 4 mm overlapping.

I have made the steel plate in "real life" and it's working nice. However I'd like to use this form in a 3D design. Now I have tried for over a week to convert the sketch to a 3D part. No sucess...

The steel plate should be 1 mm thick and there should be a 0.5 mm gap between the two ends in the overlapping.

I would be very thankful if someone can help me to make a 3D part out of it or give some hints how to do it.

In the attached ZIP file I have some files showing the layout and also a part file with the folding sketch in.
