Okay, it looks like some changes may have been made to Solidworks since I last looked at this issue.
You may look at:
And see if you can get where you need to with following that lead. I suspect that you can't accomplish what you want to do with editing the calloutformat.txt file, correctly. If I'm interpreting that variable correctly, it has to do with the fastener itself and not the hole...So, it's pulling that info from somewhere other than that text file.
As a last resort, if you only deal with a certain fastener, I suppose you could manually edit the calloutformat.txt file and put S.H.C.S into the hole callout and remove the <hw-fsttyp>. But, that is not the correct solution.

Hopefully, someone with more knowledge will post, but last I was looking at this I saw a few inquiries about how to get custom Toolbox/Hole Wizard callouts go unanswered.

So far I have not found a solution, I did a bunch of reading last friday including the links you provided. I am still searching, I will post again with what I find or do not find. You are correct that the calloutformat.txt does not solve the situation. I would have to use the <hw-fsttyp> to track the type of bolt, we use up to 3 different types here. Thanks for your help, I will post again after more research.
