The RMB "add to ordinate" option on is not available when a existing ordinate dimension is selected in SW-2016. I believe is option was available in prior releases?

Add to ordinate is available in 2016. Make sre you aren't in the smart dimension tool or this option isn't available.

Right click on a toolbar > Customize > Customization and select the Reset to Defaults button in the Shortcut customization section.
We found that if you are doing this for a hole wizard part you don't get the option if you just edit the sketch. Instead you need to edit the feature and then go under "Dimension" in the hole wizard. It's available then.
