We've had reports of this from a couple of other users and I've seen it happen myself too. We are not sure what is causing it, whether it is a disruption in the internet connection or something that is the key. Usually, I find that using ctrl+F5 (a force refresh in web browsers) fixes the problem. When you encounter it, can you test that to see if it does work? Also, if you find any key to what is causing it, please post back since we have yet to find that.
Note that we are working on a new implementation of the table of contents tree that solves some other issues and adds some enhancements so this new tree may overcome the problem if it is an issue with the older technology that we are using in the current tree.
