I started a new assembly then all my tabs have disappeared, I've restarted my computer twice and solidworks over 5 and it doesn't matter if I try to make a new assembly or open an existing one, none of the tabs or buttons populate.
I contacted my VAR and they had me reset my windows registry. The only downfall was I had to reload all of my settings however it brought all the tabs back properly. I'm not to sure what cause the issue but I followed these steps and it fixed my issue.
I'm not saying that this post is 'exactly' relative, but you never know. I just remember reading it recently and thought I'd bring it to your attention.
All simulation tabs disappeared (6Gb of simulation data made useless)
Right Click on that assembly tab and select customize CMD.
And then either select the tabs from the list OR right click on the tabs you want to show and select show tab.
You can recover your settings:

and Command Manager will be shown again.
I'd suggest just saving your file and restarting SW.
Rather than resetting the whole Solidworks registry entry, I had luck with deleting only these two:
HKEY_CURRENT_USER\Software\SolidWorks\SolidWorks 2016\User Interface\CommandManager
HKEY_CURRENT_USER\Software\SolidWorks\SolidWorks 2016\User Interface\Custom API Toolbars
This seems to bring any Command Manager customizations back to default, without affecting anything else. And for me, it brought back the Evaluate tab I was missing.
