When adding a new sheet to a existing drawing, my second sheet is blank, no titleblock
If you right click on sheet 2, or the newly added sheet, and select properties, you can choose which type of format you would like the drawing to be.
I did that several times, and even though in the preview pane the tile block was showing, and the correct file was chosen, it still comes in blank. It just started doing this. It has been working fine for several years.
Look at Tools > Options > System Options > Drawings, near the bottom "Show sheet format dialog on add new sheet". If that's checked, then SW should ask you to select the sheet format when you insert a new sheet. If that's not checked, and it sounds like yours isn't, then the new sheet will use the same sheet format as the active sheet. In your situation, I'd guess that SW can't find the active sheet's format because it's been re-named or moved since the drawing template was last saved, or your settings have changed at Options > System Options > File Locations > Sheet Formats.

The box is checked, the file listed is correct, the preview pane shows the correct title block, yet it comes in blank. I have since found out through the help files that you can cut & paste the template into a drawing by clicking on the template, using the control key, then pasting it outside the existing drawing template, which is kind-a-cool!!
What version of Solidworks are you using?
Take a look at this thread and see if it offers you any solutions.
If I'm not mistaken there is a box in the sheet format dialog box near the bottom left corner that you can select to not show the title block, lines, etc; just the blank sheet. I'm at home now and don't have SW here to double-check or get the exact wording, but you might check that. I never use it, so I could be mistaken, but it's worth checking.
I was right. See below. Maybe yours isn't checked? That would account for the behavior you described.

Did you check your "Sheet format for new sheets" setting in document propertiers->Drawing Sheets? This is new for 2014
